Job Summary

Provides clinical support and care management for patient populations at high risk for health deterioration and/or poor outcomes.  

Responsibilities and Duties

       1. Provides clinical support for medical practitioners in treatment of high-risk patient populations,
          such as patients, with diabetes or substance use disorder.

1. Assesses health risks unique to each patient.
2. Educates patients regarding disease process, treatments, and secondary prevention, including lifestyle modifications and proper use of medications and medical equipment.
3. Monitors treatment adherence and progress as directed.

1. Serves as clinical contact person, communicating with patients and responding to phone calls and messages.
2. Assesses health status and coordinates higher level of care when needed.
3. Schedules follow up appointments.
4. Arranges and tracks referrals.
5. Communicates with integrated care team members to ensure the needs of patients are addressed.

1. Receives calls and messages from patients and coordinates responses.
2. Relays patient education per procedure, including diabetic/insulin teaching.
3. Triages patients for walk-in needs and discusses with medical practitioner.
4. Assists medical assistants in performing technically difficult clinical procedures.
5. Reads TB skin tests.
6. Performs nursing procedures.
